Community Programs and Services: ANCHOR
(Addressing the Needs of the Community through Holistic, Organizational Relationships)
The Division of Community and Population Health works to systematically identify patients' health-related social needs seen in their primary care practices in Northern Manhattan and address their identified needs through referrals to local community-based social service organizations.
The Division is leveraging these efforts to standardize and expand its existing Ambulatory Care Network pre-visit screening efforts. ANCHOR systemically screens for health-related social needs and clinical risk factors. This screening helps them identify the most vulnerable patients and improve their access to preventative services. Access is improved through a combination of social and clinical interventions in the community.
