  1. Triaging the patient to the appropriate NewYork-Presbyterian hospital and unit
  2. Coordinating all administrative tasks, including bed assignment
  3. Facilitating communication between care teams
  4. Providing patient updates throughout the transfer process
  5. Arranging EMS transport, if needed
